Investigations by the Ark City Police Department and the Kansas Department of Labor concluded that Marc Coco Tapias death last December while cleaning a street sweeper could have been prevented by well-documented safety training and better maintenance of the street sweeper Tapia used for work.  Tapia was crushed between the street sweeper and a city truck.  The state determined the street sweeper was unsafe to operate due to braking issues.  The city was unable to produce any record or documentation of proper training of employees in the safe use of the street sweeper.
